OUR PRACTICE AND FORMS

NEW PATIENT What to expect of my first visit? We strive to make your first visit to our office a comfortable experience. Cheryl will welcome you and ask you to complete our required forms. You can download and print forms and bring along with you. These forms will give us important information about you and your condition. The doctor will then spend some time asking specific questions about your condition to determine if chiropractic care is right for you. If so, certain treatment will be discussed. If chiropractic care is not the answer for you, we can help refer you to get the care you need. ​ After discussing your condition, the doctor will give you a thorough examination. This will consist of testing muscle strength, reflexes and flexibility along with other neurological & orthopedic tests. This important information will allow the doctor to have a better understanding and how chiropractic can help you. If needed, the doctor may order xrays or a MRI, which would be done at an off-site facility, such as the hospital. ​ Once this is complete, you and the doctor will take a look at the overall results of the conversation and the exams. Together, you will come up with a plan of action which will begin on your very first visit. Depending on your condition, you may need to return for one or more follow-up visits.

PAYMENT POLICY Services are provided and billed by Family Chiropractic Associates. Our providers participate in some major health insurance plans, including Medicare. Please be aware that with any health plan, there may be deductibles and copays in which you are responsible for. We will do everything we can to help you afford the treatment you need and want. Forms of payment accepted by the office are cash, check, or any major credit card. We do not accept Care Credit at this time. ​

HOW TO CANCEL AN APPOINTMENT To cancel appointments, please call the office at 260-925-6686. If you don not reach the receptionist, you may leave a detailed message. If you would like to reschedule your appointment, we will return your call and give you the next available appointment.

LATE ARRIVALS AND NO SHOW POLICY We understand that there are times when you must miss an appointment due to emergencies or obligations for work or family. However, when you do not call at least four hours in advance to cancel an appointment, you may be preventing another patient from getting much-needed treatment. On the other hand, the situation may arise where another patient fails to cancel, and we are unable to schedule you for an appointment due to a seemingly "full" schedule.

LATE ARRIVALS If you happen to arrive late for an appointment, your visit may likely be shortened. Depending on how late you arrive, your doctor will determine if there is enough time for you to be seen.

NO SHOWS Anyone who either forgets or consciously chooses to forgo their scheduled appointment time will be charged $25.00 fee for the first occurrence and $45.00 for each additional occurrence. As this fee is not billed to any insurance company, the patient accepts full responsibility to pay this fee prior to scheduling another appointment.

CANCELLATIONS Appointments are in high demand and your early cancellation may give another patient the opportunity to schedule a much needed appointment. We kindly ask for at least a 4 hour notice.
